Can you believe that school is about a month away from starting. Where did the summer go? This is just a reminder to start thinking about the new school year and the paperwork that needs to be done. Don't forget to drop off any school forms that need to be signed by your doctor prior to the first day of school. Food allergies, asthma action plans, and medication forms are usually required by the school nurse for the start of school. Please make note that if you have not seen either Dr. Neaville or Dr. Saifi in the past 6 months, you will be required to schedule an appointment with them in order to have the school forms filled out and signed. Please note that the nurses are filling out a huge number of school forms and will not be able to complete them while you wait. They will get them filled out and signed by the physician within 72 hours of you dropping them off. Please let them know if you would liked to be called when they are completed and signed so you can pick them back up. We can also mail to your home address or fax them to the school (as long as we have a correct fax number of where to fax them to). Please give our office a call if you have any questions.
